How do they work?
 
Supported by a skilled facilitator, experienced in complex and sensitive investigations, and presented in a comprehensive report Facilitated Investigations follow the established Sologic Root Cause Analysis process:
                                                                                            
Step One - Gather and Understand the Data
Who was involved?  What procedures were in place?  Was the instruction clear? Was the documentation correct and was the environment suitable? What does the data (or lack of it) tell us about the issue the we need to understand?  Great analysis is built on gathering and managing the available data.
 
Step Two – Define the Problem
Create a Problem Statement to determine precisely the issue you intend to investigate or problem you need to solve.  Define the time frame of the event, the impact it had and the risks that still exist. 
 
Step Three - Visual Analysis
Visual analysis provides a systematic and objective exploration of the issue. The Sologic facilitator will lead the group through a process that enables them to explore the causes and effect of any event or issue. Guided visual analysis ensures that the team remains focused and productive and that no elements of the investigation are overlooked.
 
Step Four - Identifying and Selecting Solutions
The Facilitator will support the group in identifying potential solutions against specific causes. The team will collaborate to assess potential solutions for their effectiveness, any barriers to implementation, their cost benefit analysis as well as any wider impact on colleagues, clients or the organisation.
 
Step Five - RCA Report
Your facilitator will deliver a comprehensive report based on the outcomes of the project which will enable your organisation to make clear decisions and deliver an effective action plan based on objective findings.
